✅ This appointment calendar is for people who have completed the online intake form and provided IDs and Medical cards for all household members.

✅ If you have not yet completed the intake form and submitted valid IDs and Medi-Cal cards for all household members, please schedule an appointment through the section that includes the intake form first. Appointments scheduled without these requirements will be canceled.

✅ Missed Appointments: No-shows may not be able to schedule again. Please reschedule at least 6 hours in advance.
